Form - Opium, Coca Leaves, Opiates. This original 1953 United States Internal Revenue official Treasury Department order form for "Opium, Coca Leaves, Opiates, Etc." (Series 1948) represents a rare and historically significant piece of mid-century medical, pharmaceutical, and legal ephemera. Issued to Osborn Pharmacy of Jackson, Michigan, and made out to Walding, Kinnan & Marvin Co. of Toledo, Ohio, this duplicate order document is stamped by the District Director in Detroit, Michigan, and features handwritten entries for prescription narcotic compounds alongside a large, bold "DUPLICATE" overprint. Official US Internal Revenue / Treasury Department, dated November 16, 1953, printed paper document with Intaglio-engraved border and watermark, stamped "Osborn Pharmacy, Jackson, Mich" and numbered K493781, measures approximately 6 x 10 inches.
